Individual Recommendation Forms

The online recommendation forms are highly preferred over the Word forms. Only as a last resort should your recommenders submit the Word versions. But feel free to download the forms for your own reference.

The recommendation forms are fillable Word files. Please complete Part A and save with your full name as the new file name. Then, instruct your recommender to save a completed copy filling out Part B and email it to graduate.recommendations@wheaton.edu. Each recommendation must be received as an attachment from the recommender's email address or in a sealed envelope with the recommender’s signature over the sealed flap.
